This isn't a problem because it will show back after a few minutes and this can happen to any theme you install, not just mine.
The tumblr 'follow' and 'dashboard' button are put there by tumblr itself, not by me or any other theme designer.
It can happen that your askbox doesn't work, this is because you turned it off in your blog's settings. This has nothing to do with the theme you installed.
Check your blog setting on your blog's dashboard an if it's turned off, turn it back on. If you put '/ask' after your tumblr URL you can see if it's turned on or off.
An example would be: http://themaxdavis.tumblr.com/ask
It can happen as well that you submitbox is turned off, this has nothing to do with the theme you installed, it's because you didn't turn the function on on your blog's settings page.
Just go to your blog's settings page and turn the function on. If you put '/submit' after your tumblr URL you can see if it's turned on or off.
An example would be: http://themaxdavis.tumblr.com/submit
If you want to change your tumblr url you have to go to your blog's dashboard and click on 'settings'. There you will see an option to change your blog url. Once you've done that just scroll down and click on 'save'.
For this option you have to go to your customize page (link), scroll down and you'll see the topic 'advanced'. Click on the drop down menu and chose the amount of posts you want on your blog.
Some of my themes allow you to enter your twitter username and your tweets will show up on your blog. Before you enter your username you have to make sure your tweets aren't protected. If they're protected, you've have to login to your twitter and put that option off.
Your tweets should show up now.
Click on Edit HTML and the code that makes your theme will slide open, please don't delete things here. If something goes wrong and your theme looks all weird just reinstall the theme.
If you want to add CSS you do that between <style type="text/css"> and </style>
